This is a combination regimen consisting of four drugs—ciprofloxacin, metronidazole, prednisone, and tacrolimus—each with distinct mechanisms of action. Ciprofloxacin is a fluoroquinolone antibiotic that inhibits bacterial DNA gyrase and topoisomerase IV, leading to the disruption of bacterial DNA replication and cell death. Metronidazole is a nitroimidazole antibiotic effective against anaerobic bacteria and certain protozoa; it works by causing DNA strand breakage through the generation of reactive intermediates in susceptible organisms. Prednisone is a synthetic glucocorticoid corticosteroid that suppresses inflammation and modulates immune responses by binding to the glucocorticoid receptor, altering gene transcription. Tacrolimus is an immunosuppressant calcineurin inhibitor that prevents T-cell activation by inhibiting calcineurin phosphatase activity after binding to FK506-binding protein 12 (FKBP12). This combination may be used in complex clinical scenarios such as severe or refractory inflammatory bowel disease (e.g., Crohn’s disease), post-transplant infection prophylaxis or treatment, or other situations requiring both broad-spectrum antimicrobial coverage and potent immunosuppression[1][2][4][5][6].
