This is a combination of four active pharmaceutical ingredients—dexamethasone, dopamine, enoximone, and nitroglycerin. Each component has a distinct mechanism of action: - **Dexamethasone** is a synthetic glucocorticoid corticosteroid with potent anti-inflammatory and immunosuppressant effects. It acts by binding to the glucocorticoid receptor, modulating gene expression to suppress inflammation and immune responses[1][4][8]. - **Dopamine** is an endogenous catecholamine that acts as an agonist at dopaminergic receptors (primarily D1 and D2), as well as adrenergic receptors at higher doses. It increases cardiac output via positive inotropic effects and causes vasodilation or vasoconstriction depending on the dose. - **Enoximone** is a selective phosphodiesterase 3 (PDE3) inhibitor with positive inotropic and vasodilatory properties. It increases intracellular cAMP levels in cardiac myocytes leading to increased contractility, while also causing vascular smooth muscle relaxation[7][10]. - **Nitroglycerin** is an organic nitrate that acts as a nitric oxide donor, leading to activation of guanylate cyclase in vascular smooth muscle cells. This results in increased cGMP levels causing vasodilation—primarily venous but also arterial at higher doses. This combination would theoretically provide anti-inflammatory effects (dexamethasone), hemodynamic support through increased cardiac contractility (dopamine/enoximone), and preload/afterload reduction via vasodilation (nitroglycerin). Such combinations may be considered for acute cardiovascular compromise with inflammatory components but are not standard or widely marketed as fixed-dose products.
