Drotrecogin alfa (brand name Xigris) was a recombinant form of activated human protein C used to reduce mortality in patients with severe sepsis who had a high risk of death. It worked by inhibiting coagulation factors Va and VIIIa, providing anticoagulant, anti-inflammatory, profibrinolytic, antiapoptotic, and cytoprotective effects. It was withdrawn from the market after studies indicated it was not effective in improving outcomes in patients with sepsis. During its use, it was associated with an increased risk of bleeding. Enoxaparin (brand name Lovenox) is a low molecular weight heparin used for the prevention of deep venous thrombosis (DVT), particularly after surgery or in bedridden patients, treatment of acute DVT with or without pulmonary embolism, prevention of ischemic complications in unstable angina and non-Q-wave myocardial infarction, and treatment of acute ST-segment elevation myocardial infarction. It works by enhancing the inhibitory activity of antithrombin III. This combination, while not a formal combination therapy, would likely have an additive anticoagulant effect due to both medications affecting the coagulation system, potentially increasing bleeding risk. Given that drotrecogin alfa has been withdrawn from the market, this combination is no longer used in clinical practice.
