Overview

This is a combination of four disease-modifying therapies used in the treatment of multiple sclerosis (MS): fingolimod, natalizumab, ocrelizumab, and rituximab. Each drug has a distinct mechanism of action targeting different aspects of immune modulation: - Fingolimod is a sphingosine 1-phosphate receptor modulator that sequesters lymphocytes in lymph nodes, reducing their migration into the central nervous system. - Natalizumab is a monoclonal antibody against alpha-4 integrin that inhibits leukocyte adhesion and migration across the blood-brain barrier. - Ocrelizumab is a humanized monoclonal antibody targeting CD20 on B cells, leading to B-cell depletion. - Rituximab is a chimeric monoclonal antibody also targeting CD20 on B cells for depletion. All four agents are individually approved or used off-label for MS but are not typically combined due to overlapping immunosuppressive effects and increased risk of adverse events. There are no known clinical studies or regulatory approvals for this specific quadruple combination; its use would be considered experimental and potentially associated with significant safety concerns.
