Overview

This is a combination of three disease-modifying therapies (DMTs) used in the treatment of multiple sclerosis (MS): fingolimod, natalizumab, and ocrelizumab. Each drug has a distinct mechanism of action targeting different aspects of the immune system to reduce MS disease activity. - Fingolimod is an oral sphingosine 1-phosphate receptor modulator that sequesters lymphocytes in lymph nodes, preventing them from contributing to autoimmune reactions in the central nervous system. - Natalizumab is a monoclonal antibody that targets α4-integrin (CD49d), inhibiting leukocyte adhesion and migration across the blood-brain barrier. - Ocrelizumab is a humanized monoclonal antibody directed against CD20 on B cells, leading to their depletion. These drugs are not typically used together due to overlapping immunosuppressive effects and increased risk for infections; rather, they are often considered as sequential monotherapies or compared as alternatives for patients with relapsing forms of MS[1][5][7]. There are no approved or established regimens combining all three agents simultaneously.
